My Suicide Story is continuously seeking applicants for upcoming episodes of the series. If you are interested in sharing your story of survival, please fill out the form below. If you would like to be featured on our TikTok page, please follow the instructions below this form to send us your video.

Submit your video for TikTok

Please follow the instructions below to share your video with us. Upon receipt, your video will be reviewed and potentially posted to our TikTok account.
NOTE: By submitting your video, you grant My Suicide Story a license to use, display, and distribute your video in any and all media formats for the purposes of promotion and content creation.

Instructions

1. Position + Lighting is key. Videos can be recorded in either portrait or landscape. Please be sure to sit center to the camera and be in a well lit space with your face illuminated.

2. Tell your story. Feel free to share any relevant information that you’d like. There is no need to censor content or language.

3. Share your video with us.

   1. On your phone, visit www.wetransfer.com and click I agree in the box.

   2. Click Upload files and choose your video from your library.

   3. Once uploaded, click next.

   4. Choose the option to Send an email.

   5. Input our email address — realsuicidestories@gmail.com — in the Email to field.

   6. Enter your email address in the Your email field.

   7. Click Transfer.

   8. You will then be sent a verification code to the email you entered. Enter this code and click Verify. Once your upload begins, please be sure to stay on the transfer page until progress is complete. Be sure to keep your phone active so your screen does not shut off.
